Online forms: Launch a Legend

This initiative is about showcasing children who undertake activities that can inspire other children to have a go, but also an opportunity to recognise and celebrate your child's commitment to their passion.

The activity must be something that other children can do as a hobby, so we encourage only 'pocket money passions'.

Please complete the following questions to nominate your 'legend'!

Completion and submission of this form, declares that;

• You have read all the criteria and requirements.
• Your child is happy to participate and be filmed.
• You are happy for your child to appear on The Good Life website and You Tube. Remember these can be seen across the world and are often linked to other websites.
• You and your child are happy to be featured in the Chronicle and other related press and media.
• The Good Life involves a range a partners who also may use the scheme and film footage to promote Launch a Legend and our local children undertaking positive activities. These include: Cannock Chase Council, South Staffordshire Primary Care Trust, Cannock Chase Commissioning Consortium, Greenwood Studios, Staffordshire County Council services.

You will be notified on August 8th via email, whether you have been selected.
